1. INTRODUCTION
This Financial Policy outlines the payment terms, billing practices, and financial obligations for all patients receiving telemedicine services from Vitalized Lab LLC. By using our services, you acknowledge that you have read, understood, and agree to comply with this Financial Policy. Payment is a requirement for receiving services at Vitalized Lab LLC.
2. CASH-BASED PAYMENT MODEL
Vitalized Lab LLC operates exclusively on a cash-based payment model. We do not bill insurance companies, including Medicare or Medicaid, and we do not participate in any insurance networks. All patients are responsible for paying out of pocket for all services rendered. You are solely responsible for determining whether your health insurance covers telemedicine services and for submitting claims to your insurance provider if you choose to do so.
3. SERVICE FEES
Service fees at Vitalized Lab LLC vary based on the specific services requested and individual patient needs. There is no fixed pricing sheet. The cost of your consultation will be determined based on the nature of your visit, the complexity of your condition, and the services provided by your healthcare provider. You will be informed of the applicable service fee prior to scheduling your appointment.
Services offered include but are not limited to:
– General medicine consultations
– Wellness evaluations and health coaching
– Hormone deficiency testing and hormone therapy
– Sexual health services
– Preventative health services
– Sexually transmitted infection (STI) and herpes treatment
– Vaginal health consultations
– Reproductive health services
– Fertility consultations
– Laboratory testing coordination
– Weight loss and metabolic health services
4. PAYMENT COLLECTION AND TIMING
Payment is required prior to your telemedicine visit. Credit card information will be collected before the time of your appointment. Vitalized Lab LLC accepts payment through Authorize.net and Stripe. Your credit card will be securely processed through our payment processors in compliance with all applicable data security and privacy laws.
By providing your credit card information and scheduling an appointment, you authorize Vitalized Lab LLC to charge your card for the service fee prior to your visit.
5. PRESCRIPTION AND MEDICATION COSTS
Prescriptions, medications, and supplements prescribed during your telemedicine consultation are the responsibility of the patient and are billed separately from the consultation fee. The cost of medications and supplements will be determined by your pharmacy or provider and is not included in the service fee charged by Vitalized Lab LLC.
You are responsible for:
– Obtaining prescriptions from your pharmacy of choice
– Paying all costs associated with medications and supplements
– Verifying insurance coverage or obtaining pricing information from your pharmacy prior to filling prescriptions
Vitalized Lab LLC is not responsible for medication costs, insurance denials, or any fees charged by external pharmacies.
6. LABORATORY TESTING COSTS
Laboratory testing and coordination of lab work are not included in the consultation fee and are billed separately. If your healthcare provider recommends laboratory testing, you will be responsible for paying the laboratory fees directly to the testing facility.
You will be informed of approximate laboratory costs prior to testing when available. Laboratory fees vary based on the specific tests ordered and the testing facility used. Vitalized Lab LLC is not responsible for laboratory costs or any fees charged by external testing facilities.
7. FOLLOW-UP CONSULTATIONS
Follow-up consultations to adjust treatment, address side effects, or modify your health plan are charged separately from your initial consultation and are considered separate service visits. Follow-up visit fees will be discussed with you individually so that you are aware of all charges you may incur monthly for treatment adjustments.
Each follow-up consultation requires a separate payment prior to the visit.
8. PAYMENT METHODS ACCEPTED
Vitalized Lab LLC accepts payment through the following methods:
– Authorize.net
– Stripe
All payments are processed securely in compliance with PCI DSS standards and applicable data security regulations.
9. CANCELLATION AND NO-SHOW POLICY
Appointments must be cancelled at least 24 hours prior to the scheduled appointment time to avoid cancellation fees.
Cancellation Fee Schedule:
Cancellations made 24 hours or more before the scheduled appointment: No charge
Cancellations made less than 24 hours before the scheduled appointment: $150.00 cancellation fee
No-shows or missed appointments without cancellation: $150.00 no-show fee
Cancellation fees and no-show fees are non-refundable and will be charged to the credit card on file. If you need to cancel or reschedule your appointment,please contact us as soon as possible at hello@vitalizedlab.com or 801-742-1364.
10. REFUND POLICY
There are no refunds for services rendered. All payments for telemedicine consultations are final and non-refundable once the service has been provided. Cancellation fees and no-show fees are also non-refundable.
If you believe you have been charged in error, please contact us immediately at hello@vitalizedlab.com or 801-742 1364 to discuss the matter. We will review your account and determine the appropriate course of action on a case-by-case basis.
11. LATE PAYMENTS AND COLLECTION
Late payments are not acceptable at Vitalized Lab LLC. Payment must be received prior to your telemedicine visit. If payment is not received by the scheduled appointment time, your visit will be cancelled.
Failure to pay for services rendered may result in:
– Suspension or termination of your account
– Referral to a collection agency
– Legal action to recover outstanding balances
– Denial of future services
Late payment situations will be addressed on a case-by-case basis at our sole discretion.
12. ITEMIZED BILLING AND FINANCIAL DISCLOSURE
Prior to your telemedicine visit, you will receive an itemized breakdown of all anticipated charges, including:
– Consultation fee
– Any applicable follow-up visit fees
– Potential cancellation or no-show fees
– Information regarding prescription and laboratory costs
You will be required to review and acknowledge this Financial Policy by signing it prior to your appointment.
Your signed Financial Policy will be maintained in your medical record.
13. MEDICAL RECORDS AND DOCUMENTATION FEES
Vitalized Lab LLC does not charge patients for copies of medical records, lab results, or other medical documentation. All medical records and documentation related to your care are provided at no additional cost upon request.However, any laboratory testing or diagnostic imaging costs you have paid for directly are your property,and you are responsible for obtaining and paying for copies from the testing facility if needed.
14. DISPUTE RESOLUTION
If you dispute a charge or believe you have been billed incorrectly, you must notify Vitalized Lab LLC in writing within 30 days of the charge. Contact us at:

15. FINANCIAL RESPONSIBILITY ACKNOWLEDGMENT
By using Vitalized Lab LLC telemedicine services, you acknowledge and agree that:
– You are responsible for all charges related to your care
– Payment is required prior to your appointment
– You understand the cancellation and no-show fee policy
– You understand that prescriptions, medications, supplements, and laboratory testing are billed separately
– You have received and reviewed this Financial Policy
– You authorize Vitalized Lab LLC to charge your credit card for all services rendered
16. CHANGES TO FINANCIAL POLICY
Vitalized Lab LLC reserves the right to modify this Financial Policy at any time. Changes will be posted on our website with an updated effective date. Your continued use of our services after changes are posted constitutes your acceptance of the revised Financial Policy. We encourage you to review this policy periodically.
